Completed forms may be returned to City Hall any time between
April 16, 2012 and May 7, 2012. A copy of your current WI Sellers Permit must accompany your application.
Paid licenses will be mailed or may be picked up starting June 11, 2012.
All fees must be paid before licenses can be issued to the business.
Fees are due by 5pm Wednesday, June 27, 2012.
Any licenses remaining in the Clerk's Office must be picked up by 5pm Wednesday, June 27, 2012.

Any Business Change May Require One or More of the Following:
-Auxiliary Questionnaire & Authorization For Release of Information- Complete if there has been a change in the membership of the company. Each new member must complete each of the forms.-Schedule For Appointment of Agent & Authorization For Release of Information - Complete if there has been a change in the agent for the business.
-Alcohol License Application Supplement - Complete if there has been a change in your business plan or the premise that needs to be licensed.
-New Operator License Application
