Required Forms

The forms listed below are required by the State of Wisconsin and will need to be uploaded to the online application under state forms. Please make you sure you include all the forms below. 

•  Alcohol Beverage License Application (AB-200) 
• AB100 - Individual Questionnaire and Authorization for Release of Information - each member of the organization must complete a Questionnaire and Authorization.
• Alcohol Beverage Appointment of Agent (AB-101) and Authorization for Release of Information
• Supplement to Schedule for Appointment of Agent
• Alcohol License Application Supplement
• Addendum to Question 9 of the Original Alcohol Beverage Retail License Application 
• New Liquor License Packet Checklist

Required Information

• Current Wisconsin Seller's Permit
• Federal Employer Identification Number
• A lease or ownership papers which show possession of the premise
• Registration with the State of WI as a Corporation, LLC, etc.
